Open Access

Our Open Access Program is a service focused on you. Your needs and your family’s needs. Partnering with your physician, our nurses and social workers, specially trained to help with your needs, provide symptom management, medication oversight, support, education, and counseling on a fee-per-visit basis.

Open Access is designed to meet the needs of people with serious illness and limited prognosis who may be considering hospice services but are still receiving curative treatment such as chemotherapy or radiation. This specialty program will help if you have troublesome symptoms but have difficulty getting to the physician’s office, are not ready to consider hospice, do not qualify for homecare, or if you need assistance with the physical, emotional, and psychological distress of coping with a life-limiting illness.

An innovative approach to patient-centered care, designed to meet the needs of people with serious illness and limited prognosis who:

  • Do not qualify for home care
  • Are not ready to consider hospice
  • May be considering hospice services but are still receiving curative treatment such as chemotherapy or radiation
  • Have troublesome symptoms but have difficulty getting to the physician’s office
  • Need assistance with the physical, emotional, and psychological distress of coping with a life-limiting illness

The VNA’s Open Access program is focused on patient and family needs. Partnering with your physician, our nurses and social workers are specially trained to provide symptom management, medication oversight, support, education, and counseling on a fee-per-visit basis. Visits, including the first consultation are provided by a Registered Nurse. People experiencing financial hardship may complete a fee assessment and qualify for reduced rates or charitable funds.
