Telehealth for Cardiology
The VNA instituted its Telehealth initiative in March of 2007, and since then, has managed over 1,495 patients. The VNA is able to collect “real time” daily data through the wireless Telehealth system. This system allows for your loved ones with cardiac diseases, such as chronic heart failure (CHF), to monitor their vitals on a daily basis. This information is then electronically relayed to the VNA’s offices where the cardiac nurses can monitor and access the information remotely.
The VNA’s Telehealth service is predicated on educating our patients about self-care management and keeping them out of the hospital. Through the monitoring of the daily data that comes through with the Telehealth machine, the VNA nurses are able to catch any problems before the patient even know they exist. It is this proactive approach that allows your loved ones to remain in the comfort and safety of his or her own home during their recovery with the VNA.
