The VNA Hospice social worker can help hospice patients and their families on a multitude of levels. The VNA Hospice social workers will:
- Educate the patient and family on what to expect while receiving hospice care
- Evaluate patient needs and make recommendations to improve quality of life
- Connect patients and families to resources
- Advocate for a patient’s end-of-life care wishes
